The transformer hum I ignored for 2 years was my own wiring

Had a DSC panel in a doctor's office that hummed since the install, just wrote it off as normal. Last month the customer called about power flickering and when I finally traced it, the AC transformer was sharing a knock-out with a 120V circuit. Has anyone else chased a 'feature' that turned out to be their own mistake?

Man, that hits home in the worst way. I spent a whole afternoon once chasing a ground fault that was tripping a GFCI in a bathroom, turned out the neutral and ground were swapped at the panel from like a decade ago. I'd walked past that panel a hundred times and never thought to check my own work. The hum thing is worse though, because you literally heard it every day and just convinced yourself it was normal. It's like the wiring equivalent of ignoring a smoke alarm beeping because you think it's the neighbor's car.
