The day a 6 inch nipple nearly cost me my whole shift
Two years back I was fitting up a spool at a paper mill in Georgia and grabbed a 6 inch nipple that looked right in the rack. Torqued it down, walked away, came back after lunch and the flange gap was off by a quarter inch, so we had to cut the whole thing loose and start over. Turns out the nipple was tagged for a different job and someone set it back in the wrong bin. Five hours of rework because nobody checked the tag, and the foreman made us do a full tool box talk on it the next morning. Now I check every fitting tag before it touches the pipe. What is your worst mix up from grabbing the wrong part off the rack?
